Changing your name? To get a new Social Security card, you will need identification that shows your old name and your new name, such as a marriage certificate or divorce decree. You should purchase at least 3 copies of your recorded marriage certificate. Keep one copy in your permanent records, use the second copy for Social Security and keep the remaining one for any other government agency that might require it.

If you were born outside the United States, you also need proof of U.S. citizenship
or that you are living in the U.S. legally.

You can call Social Security at 1-800-772-1213 for more information or you can visit the SSA online. Click here to go to the U.S. Social Security Administration website and access the form for a name change.

The same instructions apply when changing your driver's license. The DMV will also require the same identification showing both your old name and your new
name. Contact your local Department of Motor Vehicles for more details.

Your employer will also need to be updated when you receive your new Social Security card and driver's license.
